Sorry for all the posts, but all of my clients have recently needed your tool.

We have a website that doesn't lock down changes for users, unlike some of my other websites. I have attached the Hardening Options PDF. Can you please let me know where the issue may be?

I have a few ideas why :)

We have a website that doesn't lock down changes for users, unlike some of my other websites.

Are you talking about the "Disable editing user properties" feature, or something else (e.g. registered users editing their own profiles)? Because this feature will only prevent users editing their own profile if their group is in the "Disable editing user properties for these groups" with an empty option like I see in your PDF being understood to mean "Super Users only".
