Mandatory information about my setup:

Joomla! version:3.1.1
PHP version: 5.3
MySQL version: 5
Host: Godaddy
Akeeba Backup version which took the backup: 3.7.7
Kickstart version used to extract the backup:3.7

Description of my issue:
I do not have an .htaccess file to alter and that is what is indicated in the literature. It also indicates to change the cookie params in the configuration.php file but they are already in line with what is indicated. I could not find an example of why the menu items, even when I create new menu items, it inserts the 'index.php' into the url. For example, If I click on contact_me from my_site, instead of getting the expected www.my_site.com/contact_me, it throws www.my_site.com/index.php/contact_me

This issue is not related with our software. That's a Joomla! configuration option found in your site's Global Configuration. You may want to click on the Help button there and heed the warnings about Search Engine Friendly URLs with URL rewriting requiring htaccess.txt to be renamed to .htaccess. Also remember that on GoDaddy any .htaccess change will take up to 30 minutes to have any effect.
