 eXtract Wizard 3.3 reports a successful dry run.
I transferred up the jpa using fetch (binary mode)
/installation/index.php certainly exists. I've cmod'd the folder and file to 777.
Database appears to have restored just fine

I'm sorry, can you be more specific?what's the error you got?
Are you trying to restore locally or on a server?
Why did you use the extract wizard and not kickstart?

Sorry, thought I put that complete in the post. A 404 error:

Not Found

The requested URL /installation/index.php was not found on this server.

I was trying to migrate the site. The jpa was ftp'd up and local to the new server, but from a different machine. Everything was in the server webroot.

I ran the wizard on my workstation to confirm the jpa was good. Otherwise, I just followed the instructions for the akeebabackup.

That's very very strange.
Can you please post a screenshot of the contents of the installation directory?
